New Patient Information
You're in great hands
We make sure all our new patients feel welcome, taken care of, and safe. During your first visit, there will be a review your medical history. For diagnosis, you will have an exam of your spine or any other symptomatic regions. Dr. Emerich will then explain any issues and help you choose options for treatment.
For a shorter time in the waiting room, please fill out the New Patient Form and bring it along with you for your first appointment.

Insurance card (if your insurance covers care)
Picture Identification Card
Any lab work that has been performed
Please be sure to dress comfortably, or bring comfortable,
loose fitting clothes appropriate for examination and treatment
